In line with Gateshead’s continuing pledge to put people and their families at the heart of everything we do and our continued expansion of outcome focussed, strength-based practice, we are looking to recruit Domiciliary Care Workers (permanent and 12 month Fixed Term Posts) to work within Gateshead Council’s Enablement Service, PRIME (People Regaining Independence by means of Enablement). This is an exciting opportunity to join a well-established and experienced team that will provide short term support to individuals across all of adult social care who may be experiencing physical or cognitive difficulties. You will work in partnership with individuals in the community, their families and other agencies who may be involved to motivate and maximise potential in those individuals so they can lead fulfilling lives, as independently as possible.
In collaboration with line managers and other professionals Domiciliary Care Workers will assist service users with weekly goal planning and accurately feedback on progress towards enablement outcomes
As a key member of the team, you will support colleagues across services to implement and review plans designed to increase independence. Examples of these individual plans include maximising independence of daily living skills including meal preparation, personal care or mobility. Your development within this is key, you will be capable at operating within a multidisciplinary environment, be highly motivated, organised and able to prioritise.
You will be flexible in your approach with a commitment to our enablement approach, operating within outcome led personalised support, based on structured individual assessments to minimise or eliminate a client’s dependency on long term social care.
