The team…
Our Enablement and Independence Services are key to supporting those who use our services to access educational and development opportunities.
The team consists of highly skilled staff who specialise in assisting people with Learning and Physical disabilities, to access activities which enabling them to achieve their personal goals and realise their full potential.
About the role….
In this role, you will play a key part in the delivering Activity Based Care Programme, which provides person-centred care following individual enablement plans, relating to Adults with Learning Disabilities, with a wide range of complex care and behaviour support needs. You will be delivering a variety of community and building based activities, geared towards the individual’s needs and aspirations -offering a high level of support to each service user, in accordance with their personal care and support plan. You will be able to support the individual to achieve agreed enablement goals, gain new or improved skills promote confidence and community engagement alongside encouraging new experiences.
You will assist in the planning, implementation, monitoring and regular reviewing of the care and enablement goals and behaviour support plans, by being actively involved with service user, carers, Activity Based Care Managers, Senior Activity Based Care Officers and a range of other key people involved in the service users care and support.
You will be responsible for the safe handling of medication, ensuring all medication is administered as prescribed and in line with policy, seeking advice and guidance as needed, helping to safeguard the individual in your care.
You will be expected to complete administrative duties within the service ensuring records are maintained to a high standard. You will deliver good quality, safe working practices and adhere to the quality assurance processes of the service, adhering to the established Quality Assurance Framework and best practice.
You will be expected to work to meet the changing needs of the service, and have a flexible approach towards your working hours when required, which may include working early mornings, evening and weekends.
